BCC originates from stem cells in the intrafollicular epidermis or hair follicle infundibulum undergoing ultraviolet light (UV)-induced mutagenesis. It has been estimated that 30 to 40% of patients with BCC will develop one or more new lesions over the next 10 years.

Histologically, BCCs can be classified into four major subtypes: nodular, morpheaform, metatypical and superficial.
